Chronic care management
Living with diabetes, high blood pressure or another long-term condition takes steady support. Regular virtual visits help you stay on track between doctor's appointments.
What chronic care management is
Instead of one visit now and then, you have a plan and regular check-ins: your provider looks at your home readings and lab results, reviews your medicines and helps with the everyday parts – food, activity, sleep and stress.
Conditions it can help with
- Type 2 diabetes and prediabetes
- High blood pressure
- High cholesterol
- Asthma and COPD (alongside your regular doctor)
When to get help straight away
Very high or very low blood sugar with confusion, a blood pressure reading with chest pain, headache or vision changes, or trouble breathing – call 911. A virtual visit is not an emergency service. For chest pain, trouble breathing, severe bleeding, signs of a stroke or a serious injury, call 911 or go to the nearest emergency room. If you are thinking about harming yourself, call or text 988 (Suicide & Crisis Lifeline) at any time.
Questions about chronic care
Can I get chronic care management at CheckMD today?
We are adding it with a specialist care partner, so availability is still limited. Send us a message before you sign up and we will tell you what is possible for you.
Does it replace my regular doctor?
No. It works alongside your doctor, and your visit notes can be shared with them.
Do I need special equipment?
A home blood pressure monitor or glucose meter helps. Your provider will tell you what is useful.
