Remote patient monitoring (RPM)

Connected devices send your blood pressure, blood sugar or weight readings to your care team, so problems are spotted early – without extra trips.

How remote patient monitoring works

  • You get a connected device, such as a blood pressure cuff, glucose meter or scale
  • You take readings at home as your provider asks
  • The readings go securely to your care team
  • Your care team checks them and contacts you when something needs attention

Who it helps

  • People with high blood pressure, diabetes or heart failure
  • People recovering after a hospital stay
  • Anyone whose provider wants to see readings between visits

It is not an emergency service

Readings are checked regularly, not every minute. If you feel very unwell, don't wait for a call. A virtual visit is not an emergency service. For chest pain, trouble breathing, severe bleeding, signs of a stroke or a serious injury, call 911 or go to the nearest emergency room. If you are thinking about harming yourself, call or text 988 (Suicide & Crisis Lifeline) at any time.
